Transitional Care Management Launch May 11, 2017
Please join us for an informational session on Transitional Care Management and how Saint Francis Healthcare Partners Population Health & Care Coordination Department can help support your office, as well as the patients you serve.
Transitional Care Management services are provided to patients who transition back into a community setting such as home, an assisted living, and/or rest home following a qualifying inpatient/observational admission. Patients must be transitioning from an Acute Care Hospital, a Psychiatric Hospital, a Long-Term Care Hospital, a Skilled Nursing Facility, or an Inpatient Rehabilitation hospital.
The informational session will cover topics such as:
- What is Transitional Care Management (TCM) coding?
- Who can provide TCM services?
- What are the CMS requirements that must be achieved to bill for TCM services?
- What are the benefits to documenting and coding a TCM?
- How the SFHCP Care Coordination staff will support providers and their practices with TCM services?
The informational session will be held on Thursday, May 11, 2017, from 12:00 pm – 1:00 pm at 95 Woodland Street, Conference Room A, Hartford, CT. For those of you unable to attend in person, we will provide the option to join the meeting via Webinar.
