Town of Manchester Senior Center Health Fair

MANCHESTER, CONN. – On Thursday, May 4th, members of the SFHCP Population Health Management Team Participated in the Town of Manchester Senior Center Health Fair.

The participation evolved as a joint effort with the Saint Francis Hospital COPD Program to raise awareness about chronic obstructive pulmonary disease, as well as the services and support Population Health Management team members offer. Lifestyle changes and behavioral modifications including managing stress and anxiety, healthy eating, exercise, and smoking cessation was promoted.

The Population Health Management Community Wellness Program Events Committee meets monthly to identify, organize, coordinate and promote SFHCP Population Health Management services via community outreach in the community. Our next community event is scheduled this summer in Bloomfield, Conn.
