    SFHCP Eligibility Program 2017

    Saint Francis Healthcare Members:
    Your peers on the Provider Alignment Workgroup of the Roadmap to Value project have endorsed a set of Distribution Eligibility Criteria. The purpose of the Distribution Eligibility Criteria is to establish a method for driving performance in our value-based contracts. This will be the basis of determining an individual provider’s eligibility to participate in any distribution determined by the Board of Directors for the 2017 contract year performance.
    In this, the first year, we intend to make the Distribution Eligibility Criteria achievable and focused on driving engagement. In subsequent years, it is the intent of the Saint Francis Healthcare Partners Board of Directors to begin to measure additional criteria that address quality, cost and utilization.
    The essential elements of the program include:
    1. Citizenship 2. Education 3. Communication 4. Patient centeredness
    Providers must complete certain requirements in each category in order to be eligible for distributed funds. Please see the attached documents for the criteria listings, examples of improving patient access, and an FAQ sheet.
