SFHCP’s Population Health Team Guest Lecturers at University of Saint Joseph
WEST HARTFORD, CONN. – On November 2, Khadija Poitras-Rhea, Executive Director and Malone Smith, Project Manager from the Saint Francis Healthcare Partners Population Health Management team were guest lecturers at the University of Saint Joseph Senior Seminar course for students completing certification in Healthcare Coordination.
The course is taught by Katie S. Martin, Ph.D., Director, Public Health Program.
Khadija and Malone provided students with an overview of different care plan formats and interventions used to provide effective care coordination services. Students reviewed the development of health related SMART goals with patients; how to maintain healthy and professional boundaries and how to identify community resources to support patient-centered goals. Megan Casey is currently an intern with Saint Francis Healthcare Partners this semester and will be graduating in December with her B.S. in Public Health and Healthcare Coordination Certificate.
