SFHCP Participates in LiveWell!TM Chronic Disease Self-Management Workshop

HARTFORD, CONN. – Saint Francis Healthcare Partners (SFHCP) and LiveWell!TM teamed  up for a seasonal workshop for our patients.  The LiveWell!TM Chronic Disease Self-Management Workshop is a 6-week evidence-based program developed by Stanford University Patient Education Research Center.  Angie Van Ostrand, Community Healthcare Coach at SFHCP, along with Sarah T. Gauger and Erin Soli, from CT State Department on Aging, facilitated this workshop series from September 19th through October 24th. The workshop provided participants with the tools and techniques to help better manage their ongoing chronic conditions & health issues such as arthritis, high blood pressure, heart disease, chronic pain, anxiety and depression. Participants were successful in setting their own action plans to help communicate their health concerns and goals with their family, friends and health care providers.
