Population Health Management > Patient/Community Education Program

Community Healthcare Navigators and CommunityHealthcare Coaches are available to provide individualized and/or group education to patients on a variety of topics including; chronic condition management, diet and exercise, navigating healthcare systems and accessing community resources. Healthcare professionals such as physicians, case managers and social workers may refer patients to the team to initiate further education. Community Healthcare Coach or Community Healthcare Navigator assessments may also identify patients who would benefit from in-depth healthcare education as an intervention to support patient engagement and self-care management of their chronic disease.

The team is also active within the Greater-Hartford community by providing basic health information, healthcare screenings and resources geared toward improving the overall health of the population. The team partners with local community action groups to participate in health fairs, food/clothing drives and other community-based programs within the Hartford area.
