SOHO HEALTH Celebrates 25 Years

We are celebrating our 25th Anniversary! SOHO HEALTH was founded in 1993.

We have grown into a cohesive network of independent healthcare practitioners, Saint Francis Hospital and Medical Center and other facilities of Trinity Health Of New England all working together to create the future of healthcare in our region.

The active involvement of our physician owners has been critical to our success.  We count more than 750 primary & specialty care physicians and over 325 clinicians as members.

We are a strong and pioneering organization that continues to develop partnerships, programs and innovations to reduce the total cost of care, improve the health of our patients and makes us the network of choice for high quality providers.

Congratulations to all who have made this journey possible. SFHCP is stronger because of all of you!

Skilled Nursing Facility

We are pleased to share some initial successes from our first quarter of the SNF 3 Day Rule Waiver Program. This program is available to SFHCP through our participation in MSSP Track 1+. The waiver allows patients to be transferred to a Skilled Nursing Facility from an inpatient stay of less than three days, Observation Status, the Emergency Department or a provider’s office. We initially projected 36 waiver admissions for the year.

We have already admitted 27 patients, 75% of our annual target within the first three months! Our first community admission was by Nanette Alexander, APRN at Prime Healthcare. She attended a provider education session where she learned about the waiver program and contacted our office the next day with a referral. We are so excited about this program and the positive impact it will have for our patients and on our Medicare costs. For more information on the program, please visit our website.

Simsbury Networking @ Night

SOHO HEALTH hosted the third Physician Networking @ Night event on March 15th at the Simsbury Inn. Nearly 100 people attended the event including 26 PCPs and 41 specialists.

The event kicked off with cocktails, hors d’oeuvres, and socializing. Dr. Kimmel addressed the crowd and stressed the importance of managing our patients within the SOHO HEALTH network to improve quality of care and financial performance. “Speed Networking” ensued as groups of specialists rotated between tables of PCPs in order to establish new connections or reinforce old ones.

SOHO HEALTH will host a Networking @ Night event for Practice Managers on May 16th at the Chowder Pot restaurant in Hartford. Registration is now open on Eventbrite.

SOHO HEALTH was recently featured in the Hartford Business Journal on March 26, 2017.  The article is about SOHO HEALTH’s successful Networking @ Night Events open to SOHO HEALTH Providers in our network to help strengthen ties and in network referrals.

Risk Management Education Sessions Announced!

As the healthcare risk landscape continues to evolve, I am pleased to announce that SOHO HEALTH will be continuing its partnership with Med-IQ, a leading national provider of risk management education and consulting.  Med-IQ provides SOHO HEALTH members with CME-accredited live and online education programs that have proven to mitigate risk, while helping to improve patient safety and outcomes.

This program is provided as a benefit to all SOHO HEALTH providers and practice managers at no additional cost.  In addition, with completion of the Med-IQ program, up to 15% in premium credits are available to members who participate in the CAP-PG insurance program.  (For additional information, please contact Kathy Strauch at 860-714-1245 or kstrauch@stfranciscare.org.)

Aetna Enhances ACO Product in Connecticut

Aetna and Trinity Health Of New England previously announced an enhanced accountable care product, Aetna Whole HealthSM Value Care Alliance and Trinity Health Of New England. The new product will provide employers with a value-based health care option designed to increase quality and improve the patient experience while driving efficiency and controlling costs.

Employers who choose Aetna Whole HealthSM for their health plan will now be able to leverage Trinity Health Of New England’s more than 900 physicians in more than 200 practices and four hospitals, including Saint Francis Hospital and Medical Center in Hartford, Saint Mary’s Hospital in Waterbury, Johnson Memorial Hospital in Stafford Springs and Mount Sinai Rehabilitation Hospital in Hartford.

Cigna Health Fairs

SOHO HEALTH joins Cigna at a Health Fair for the City Of Hartford Employees. The SOHO HEALTH team provided valuable information to employees regarding Health Coaching services and local urgent care options. We also connected patients to Primary Care and Specialist providers who are accepting new patients.
