How do I communicate with patients and families? May 02, 2018

Please verify eligibility prior to any discussion of the waiver with patients and families. The patient is not required to wait in your office while we complete this process. Once you have confirmed eligibility the patient may go home while we complete the required paperwork and locate a SNF bed. You should provide the patient with the list of approved facilities and let them know that our Waiver Coordinator will be in touch. We will ask the patient/family to select  their top three facilities so that we may begin the bed search.  Once they have selected the facilities, we will make referrals and send the required documentation. When a facility accepts the patient we will contact the patient/ family to coordinate and notify your office of the placement.
